Using a standard, evidence-based screening tool, CBC Screeners assess individuals for unmet social needs such as food insecurity, housing instability, transportation barriers, and other social risk factors that impact health and well-being. Following screening, trained navigators work one-on-one with participants to connect them to appropriate community-based organizations, public benefits, and social services within the SCN and broader local ecosystem. Navigators offer culturally responsive support, assist with referrals and applications, and provide follow-up to ensure connections are successfully completed.
