Learning Communities

The health and social care needs of those we serve are complex and constantly evolving.

 

CBC’s Learning Communities bring together diverse sets of clinical and social service providers, stakeholder partners and consumers both within and across programs, agencies, and systems to align around shared goals and rise to meet these challenges. Uniquely tailored to specific populations and gaps in care, each learning community fosters connection, open communication, and innovation, upholding joint accountability and commitment to working through and solving problems together.

CBC’s Learning Communities take shape in various formats and are often embedded in a broader Quality Improvement framework or program. Participants typically meet regularly over a defined time period to work towards a common goal. Examples of past and present CBC Learning Communities include:
