Coordinated Behavioral Care (CBC) transforms lives through our innovative network of New York City's top nonprofit health and human services agencies. Serving over 150,000 people every year, we provide vital services including behavioral health, primary care, care coordination, housing, and social care. Our client-centered, data-driven approaches ensure we deliver the best practices to address the most challenging issues faced by individuals and families.

CBC unites 77 agencies under the CBC Independent Practice Association and Health Home, serving adults and children across New York City, Long Island, and lower Westchester. We collaborate with the broader healthcare system, payors, government, and communities to develop and implement effective solutions. 

Our unique approach creates a sophisticated care ecosystem that improves outcomes. Our centralized multidisciplinary team, including peer support, aligns the network around shared values, operates a centralized intake system, coordinates outreach and services, provides clinical support, conducts case conferences and training, and drives quality improvement through data-informed strategies.
