Safe Options Support (SOS)

In response to the persistent challenge of homelessness, the Safe Options Support (SOS) program was established through a collaboration between CBC and the New York State Office of Mental Health. This groundbreaking multidisciplinary mobile team model serves individuals experiencing homelessness on the streets and in public transit locations, providing both immediate and long-term support to facilitate their transition from homelessness to stable housing. 

The program emphasizes cross-system collaboration, enabling effective data sharing, care plan coordination, and access to vital community-based resources. By integrating a person-centered approach with system-wide cooperation, SOS addresses the complexities of homelessness with innovation and compassion.

Services

Intensive Outreach and Engagement: Proactive, consistent efforts to build trust and connect with individuals in need. 

Immediate Needs Resolution: Providing essential items such as food, water, clothing, blankets, and hygiene supplies.

Documentation Assistance: Helping individuals obtain identification and other necessary documents to access benefits.

Comprehensive Referrals: Connecting individuals to behavioral health and primary care services, as well as social services like legal aid, employment assistance, and financial counseling.

Housing Placement: Supporting individuals through the process of securing stable housing.
