Our Legacy: Building a Healthier Community Together

 

The CBC Network brings together some of New York City’s most esteemed community-based providers, all united by a shared commitment to improving the health and well-being of individuals facing behavioral and physical health challenges, often compounded by complex social disadvantages. With decades of experience, innovation, and deep community ties, each CBC member agency and network provider plays a crucial role in advancing this mission.

Founded with the vision of creating a cohesive network within the broader healthcare system, CBC has continually evolved. Our history reflects a progressive journey toward becoming a clinically integrated Independent Practice Association (IPA), equipped with comprehensive administrative, financial, and clinical support functions. This robust infrastructure facilitates the management of complex relationships among network agencies, payors, government entities, communities, and other key stakeholders.

2011

Since our inception in 2011, CBC has been at the forefront of behavioral health network development, pioneering innovative program designs, leveraging data analytics to drive health outcomes, and fostering cross-sector collaborations to enhance community health and wellness. Established by forward-thinking NYC non-profit behavioral health organizations, CBC was created to meaningfully participate in New York State’s Medicaid Redesign initiative, addressing health inequities and ensuring comprehensive access to care.

2012 – 2014
2012 – 2014

A key milestone was the establishment of one of New York City’s largest Health Home in 2012, which currently serves over 25,000 adult and child Medicaid beneficiaries. As our network matured, we realized the vision of a citywide, integrated service delivery system in 2014 with the incorporation of the IPA. This brought together over 70 agencies specializing in behavioral health, primary care, housing, and social services, streamlining contracting processes with managed care organizations and enhancing service delivery.

One of our hallmark programs, Pathway Home™, launched in 2014, exemplifies CBC’s innovative approach. Initially funded by a New York State grant, this award-winning program provides mobile, time-limited services for adults with serious mental illness transitioning from hospital to community care. Pathway Home™ has expanded significantly, growing from five to 16 teams, and continues to be a critical component in the NYS System of Care.

In 2017, CBC IPA was awarded the OMH-OASAS Behavioral Health Care Collaborative (BHCC) VBP Readiness Grant, expanding our network to include over 50 providers and affiliates, covering 98,000 Medicaid lives. This period also saw the expansion of our governance structure and the initiation of Project Connect, which connects individuals involved in the criminal justice system with health home services.

2018-2019

The establishment of the Innovations Hub in 2018 further solidified our commitment to advancing healthcare. This initiative expanded Pathway Home™ and launched new programs like the Community Health Hot-spotting Intervention Program (CHHIP). Our inaugural Innovations Conference in 2019 brought together over 200 participants, sharing best practices and exploring emerging approaches to care, including the development of a joint IT Data Analytics Business Intelligence platform (DABI) with Arcadia.

CBC’s continued growth and innovation are evident in the creation of the CBC Training Institute in 2020, a multidisciplinary resource for training behavioral health staff and consumers across New York State. In response to the global pandemic, CBC secured emergency SAMHSA grant funding to address the unique challenges posed by COVID-19, ensuring access to testing, education, and social support services in hotspot areas.

As the overdose crisis escalated, CBC played a pivotal role in launching the Citywide Addiction Support Network (CASN), a collaborative effort among 21 NYC-based agencies and NYC Health + Hospitals, offering comprehensive addiction services. This initiative, funded by OASAS, is reshaping prevention, treatment, and recovery services for opioid and stimulant use disorders across the city.

In partnership with NYC Health + Hospitals and HealthFirst, CBC has expanded community-based care transition teams to improve patient outcomes and reduce costs. These teams, employed by CBC Network agencies, benefit from CBC’s comprehensive administrative and clinical support.

In April 2022, CBC led the launch of the Safe Options Support (SOS) program, a collaborative effort with the New York State Office of Mental Health, the NYS Governor, and NYC non-profit organizations. This initiative deploys 14 teams to provide housing and essential services to individuals experiencing street homelessness, achieving remarkable results and setting a new standard for statewide initiatives.

In 2022, CBC’s Health Home Care Management Program also expanded to include Nasssau, Suffolk and Westchester counties.

CBC partnered with Health and Hospitals Housing for Health Program to expedite housing their patients housing.

Today, CBC stands as a leader in the field, with a solid foundation as a Clinically Integrated Independent Practice Association and a Lead Health Home. Our journey continues as we innovate and collaborate to create healthier communities across New York City.
