Successful transitions from inpatient care require navigating complex and often fragmented systems. Pathway Home is designed to ease this process with intensive, in-person support. CBC serves as the single point of contact for agency-based multidisciplinary teams, ensuring seamless coordination and care.

These dedicated teams maintain small caseloads, allowing for flexible, individualized interventions. The frequency, duration, and intensity of services are adjusted to meet the unique needs of each person in their community. With the capacity for rapid response, the teams are equipped to provide critical, time-sensitive support during this pivotal transition.

Coordinated Behavioral Care IPA, Inc. (CBC) and Healthfirst Health Plan, Inc. (Healthfirst) have launched a new Pathway Home™ (PH) Team that will offer individualized, high-touch, care transition services for certain Healthfirst Medicare and Medicare/Medicaid Dual Eligible and Medicaid Advantage Plus members residing in the Bronx and Manhattan.

CBC and One City Health Pathway Home Partnership

NYC Health + Hospitals’ Office of Behavioral Health, in partnership with OneCity Health, today announced the launch of Pathway Home, an innovative care transition program created by Coordinated Behavioral Care (CBC), to help individuals with mental health needs transition from short-term health services to ongoing community-based care.

CBC’s Pathway Home wins the 2019 Case Management Society of America (CMSA) Foundation’s Case Management Practice Improvement Award

The Pathway Home™ Program was awarded the 2019 Case Management Practice Improvement Award, an award sponsored by the CMSA Foundation. The CMSA Foundation presented the award on June 13, 2019 during the 2019 CMSA Annual Conference in Las Vegas. CBC Senior Director Barry Granek accepted the award on behalf of the Pathway Home programs and gave the acceptance speech to approximately 2,000 conference attendees.
