The Pathway Home™ Program was awarded the 2019 Case Management Practice Improvement Award, an award sponsored by the CMSA Foundation. The CMSA Foundation presented the award on June 13, 2019 during the 2019 CMSA Annual Conference in Las Vegas. CBC Senior Director Barry Granek accepted the award on behalf of the Pathway Home programs and gave the acceptance speech to approximately 2,000 conference attendees.
In 2014, CBC created a care transition program called Pathway Home™, with the support and funding from the New York State Office of Mental Health, whose aims was to improve the reintegration of individuals back into their community after an extended hospital stay as well as overall health outcomes and life satisfaction.
The CMSA Foundation created the Case Management Practice Improvement Award as an opportunity for case managers to be recognized by their peers, demonstrating innovation and the value case management brings to the populations we serve. The award recognizes a program that uses finding from a quality and performance improvement initiative that advances case management practice and improves outcomes. Only one Case Management Practice Improvement Award is given each year.
